Survivorship

We developed our Survivorship Program The Journey Ahead to guide you and your family along your cancer journey. We consider you a survivor from the time of your diagnosis through the span of your life. It is important that we give you the best quality of life possible. We will be assessing for side effects throughout your treatment and beyond to make sure we are addressing all your needs. You will be given a Needs Assessment at varying times throughout your treatment and recovery period. The Needs Assessment will be able to track any new and resolving issues you have along your survivorship journey.

The completion of your treatment can be a very stressful time. You will probably have many questions and concerns regarding your future care. We want to help make your transition from active treatment to follow-up care as smooth as possible. Communication is key to making sure all your healthcare needs are met. A treatment summary and care plan will be given to you upon completion of your treatment. The treatment summary is an overview of all therapies you have had while undergoing treatment for cancer as well as past medical history and medication list. The care plan is a detailed plan of action for you. The care plan will address your future healthcare needs. A list of helpful resources in the community will be made available to you tailored to your specific needs. The care plan and treatment summary are yours to keep and will be used to help communicate your specific needs to healthcare providers in the community.
